I hereby submit my/my child's registration to the WDU 24th Annual National Championship. I agree to waive all claims against any person connected with this event for injuries I may sustain and assume all responsibility for all my actions in connection with this event. I understand there are rules and regulations associated with this event and I will abide by said rules and regulations. If I do not abide by these rules, Success Martial Arts Inc. reserves the right to have you leave with no refund of entrance fees. I also understand that any pictures and/or videotape of me participating in said event may be used for publicity without compensation
